“This questionnaire helps us evaluate our distance courses (online, videocassette and ITV)  in order to improve our programs and services. It is NOT AN EVALUATION OF THE INSTRUCTOR.  If you have comments in that regard, they should be directed to the division associate deans and/or through standard evaluation forms.
